• The platform that works best and is comfortable for you will need to be established. Dr. Carson prefers DOXY.ME as that just requires you to click a link, however if a different platform needs to be used OTN and FaceTime also work. If video calling is not an option, a phone call can be used as well

• If you have a list of concerns please narrow it down to 3 concerns to discuss with Dr. Carson

• Make sure you are in an area of your house where the WIFI connection is strongest to avoid connection issues

• Appointments will be scheduled in 15 or 30 minute time slots and you will given an appointment time

• We ask that you are punctual and prepared to talk to Dr. Carson at your assigned time slot

Informed verbal consent was obtained from this patient to communicate and provide care using virtual and other telecommunications tools. This patient has been explained the risks related to unauthorized disclosure or interception of personal health information and steps they can take to help protect their information. We have discussed that care provided through video or audio communication cannot replace the need for physical examination or an in person visit for some disorders or urgent problems and patient understands the need to seek urgent care in an Emergency Department as necessary.
