Please answer the screening questions truthfully as we are trying to protect all of our patients, staff, and doctors!

Please ensure we have an updated cellphone number to contact you

You will call the office from your car once you arrive, please wait in your car and do not come to the door, until we ask you to come in.

These screening questions still should be considered:

-Have you been confirmed or had close contact with someone who has been confirmed with a case of COVID-19?

Do you have any of the following symptoms?
Fever
New onset of cough
Worsening chronic cough
Shortness of breath
Difficulty breathing
Sore Throat
Difficulty swallowing
Decrease or loss of sense of taste or smell
Chills
Headaches
Unexplained fatigue/ muscle aches
Nausea / Vomiting, diarrhea, abdominal pain
Pink eye
Runny Nose/ Nasal congestion without other known cause

ON VISIT

Please bring shorts/T-shirt/tank top for more convenient physical examinations.

Please note that patients should expect to be seen by medical students or residents as well as Dr. Carson as this is a teaching facility.
